How To Schedule

Step 1 - Obtain a Physician's Order

This should be written using the term “FEES procedure' or 'Endoscopic Swallow Study' as part of the order.

Step 2 - Complete DV 360 “Patient Intake Form”

Step 3 - Obtain signed consent using DV360 “Patient Consent Form”

Step 4 - Fax the completed physician's order, patient intake form and consent form to DV 360.

Traditionally the facility SLP, rehab director, or DON initiates the referral.

Step 5 - DV 360 will contact your facility to schedule FEES service

We can be there in 24-72 hrs, but typically the facility SLP prefers to schedule a week out.
